Title: Medical Social Worker (Bilingual, Vietnamese Speaking)

Reports To: Clinical Supervisor

POSITION SUMMARY

The Medical Social Worker assists patient and caregiver(s) in acceptance/coping and understanding significant social/emotional and financial problems related to the patient’s illness and ability to comprehend and follow the treatment plan. We are looking for a Bilingual candidate that is fluent in Vietnamese

QUALIFICATIONS

1.Bachelor’s Degree from a school of social work accredited and approved by the Council on Social Work Education.

2.Possesses a minimum of one (1) year recent clinical experience in a health care setting preferably home health or geriatrics preferred.

3.Current CPR Certification

4. Is able to walk, bend, stoop, and lift objects up to 25 lbs.

5. Current and satisfactory report on pre-employment physical examination including Mantoux TB Test or chest X-ray as required by Agency policies and procedures.

6. Has excellent judgment and knowledge of current therapy practices, with demonstrated exemplary clinical, analytical and documentation skills.

7. Is fluent in English.

8. Is self-directed and able to work with little supervision and has good organizational skills.

9. Must be a licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working order.

10. Works with the family.

11. Acts as a consultant to other agency personnel.

12. Assists the patient and family to understand, accept and follow the recommendations and services that are planned to restore the patient to optimal social and physical adjustment within his/her capacity.

13. Assists the patient and families with personal and environmental difficulties, which may otherwise predispose toward illness or interfere with obtaining maximum benefits from medical care.

14. Refers patients/families to appropriate care, resource and program agencies.

15. Assists patients and families to utilize family and community agencies.

16. Advises, counsels and instructs family in patient’s social and emotional needs.

17. Assists team members in understanding the significant social and emotional factors related to the patient’s health problems.

18. Documents patient intervention and response to intervention accurately, using established guidelines.

19. Submits all paperwork (including progress summary) within time guidelines and is completed according to Agency policy and procedure.

20. Regularly participates in interdisciplinary planning, case conferences and staff meetings as required.

21. Participates in Performance Improvement activities including but not limited to clinical record review, quality indicator monitoring, and quality management committee.

22. Actively participates in Agency in-service programs and/or provides in-service related to MSW services as requested.

23. Maintains confidentiality of patient information at all times. Maintains/conserves confidentiality of patient and agency information at all times to HIPPA regulations.

24. Must participate in professional continuing education and maintain active license in the State of California.

25. Conforms to all agency policies and procedures.

Job Type: Full-time

Pay: $50,000.00 per year
